This form is to be used by one person from each book group to request copies for an entire group. Individuals should contact their book group leader to use this form, or request their own copy through the catalog.

PLEASE PLACE REQUESTS AT LEAST 2 WEEKS BEFORE THE PICKUP DATE BUT NOT MORE THAN 1 MONTH IN ADVANCE. WE CANNOT SUPPLY HIGH-DEMAND BOOKS, SUCH AS CURRENT BEST SELLERS AND TITLES RECENTLY ADAPTED TO FILM. PLEASE CHECK THE MINUTEMAN CATALOG TO CHECK THE AVAILABILITY OF A TITLE BEFORE SUBMITTING YOUR REQUEST.
